Scan-to-form lead capture

Instead of redirecting a scan straight to a website, send it to a hosted form. Use this to collect leads, registrations, feedback, or warranty registrations directly from a printed QR code. Available on Growth and above.

How it works

A scan-to-form QR points at an ExaRoutes-hosted form page rather than an external URL. The visitor fills it in on their phone, and the submission lands in your dashboard. Nothing to host, no website required.

Build the form

  1. Go to QR codes → Forms and click New form.
  2. Add your fields. Each field has a type (short text, long text, email, phone, dropdown, checkbox) and can be marked required.
  3. Set the post-submit message visitors see after they send the form.
  4. Save the form. It gets a hosted URL of the shape qr.exaroutes.com/f/<form-id>.

Point a QR at the form

When you create or edit a dynamic QR, choose the form as its destination. Print the QR as normal. Every scan now opens the form.

Where submissions go

  • Dashboard: open the form to see every submission, newest first.
  • CSV export: download all submissions for a form from the same page.
  • Webhook: subscribe to the form.submitted event under Settings → Integrations to push each submission to your CRM, a spreadsheet, or Slack in real time.

Spam protection

Hosted forms are protected by a Cloudflare Turnstile challenge, so you get human submissions without a visible CAPTCHA for most visitors.
